Each set comes with two tags, chain w/ extender, and two silencers (you choose the color).
 
Each line will allow up to 17 letters and spaces 
 
Common lay out is line by line:
Last Name
First Name/Middle Initial
DOD ID
Blood Type
Religion 
 
BUT you can put whatever you want, as long as the spaces and characters don't go over the 17 space/character limit.
